Participant Information

By signing this agreement, I acknowledge that I am voluntarily participating, or allowing my child to participate, in activities offered by Art of Takedowns Judo Academy ("AOT"), including but not limited to Judo, No-Gi Judo, grappling, wrestling, self-defense, fitness training, conditioning, open mat sessions, competitions, seminars, private lessons, demonstrations, and any related activities.

Assumption of Risk

I understand that martial arts and athletic activities involve inherent risks that cannot be eliminated regardless of the level of supervision, instruction, or safety precautions.

These risks include, but are not limited to:

• Falls and impacts
• Throws and takedowns
• Joint locks and grappling techniques
• Strains, sprains, bruises, cuts, and abrasions
• Broken bones
• Concussions and other head injuries
• Neck, back, shoulder, knee, and other joint injuries
• Permanent disability
• Serious illness
• Death

I understand that injuries may occur through contact with instructors, training partners, equipment, or other participants.

I voluntarily assume all known and unknown risks associated with participation.

Medical Responsibility

I certify that I (or my child) am physically able to participate in martial arts training.

I understand it is my responsibility to consult with a physician regarding any medical condition that could affect safe participation.

I agree to immediately inform an instructor of any injury, illness, medical condition, or limitation that may affect participation.

If emergency medical treatment becomes necessary, I authorize Art of Takedowns Judo Academy to obtain emergency medical care if I cannot be reached. I understand I am solely responsible for any medical expenses incurred.

Release of Liability

To the fullest extent permitted under California law, I release and forever discharge Art of Takedowns Judo Academy, its owner, instructors, assistant instructors, volunteers, employees, members, affiliates, landlords, and representatives from any and all claims, demands, causes of action, damages, losses, or liability arising out of or related to participation in academy activities, including those resulting from ordinary negligence.

This release does not apply to conduct that constitutes gross negligence or willful misconduct where prohibited by law.

Minor Participants

If signing on behalf of a minor, I certify that:

• I am the child's parent or legal guardian.
• I have the legal authority to sign this agreement.
• I understand the risks associated with martial arts participation.
• I accept full responsibility for the minor's participation.
• I agree to all terms of this waiver on behalf of the minor.

Safety Rules

I agree that I (or my child) will:

• Follow all instructor directions.
• Practice good sportsmanship and respect toward others.
• Wear appropriate training attire.
• Remove jewelry and other hazardous items before training.
• Maintain good personal hygiene.
• Report injuries immediately.
• Refrain from participating while under the influence of alcohol or drugs.
• Stay home when experiencing symptoms of contagious illness, including fever, vomiting, diarrhea, or other symptoms that could place others at risk.

I understand that failure to follow academy safety rules may result in removal from class without refund.

Personal Property

Art of Takedowns Judo Academy is not responsible for lost, stolen, or damaged personal property brought onto the premises.

Photography & Media Release

I grant Art of Takedowns Judo Academy permission to photograph or record me (or my child) during classes, events, competitions, or academy activities.

I understand these images or videos may be used for educational, promotional, advertising, social media, or website purposes without compensation.

If I do not wish to be photographed or recorded, I will notify the academy in writing before participation.

Membership & Participation

I understand that tuition, membership policies, billing terms, attendance policies, and cancellation procedures are governed by my membership agreement with the academy and are separate from this waiver.

Signing this waiver does not guarantee membership or continued participation.

Acknowledgment

I certify that:

• I have carefully read this entire agreement.
• I fully understand its contents.
• I understand I am giving up certain legal rights by signing it.
• I sign this agreement freely and voluntarily without coercion.
• This agreement shall remain in effect for all current and future participation in Art of Takedowns Judo Academy activities unless revoked in writing and accepted by the academy.

By signing below, I acknowledge that I have read, understood, and agree to all terms contained in this Waiver, Release of Liability, and Assumption of Risk Agreement.
